Recurrent UTIs: A Naturopath’s Approach

Recurrent UTIs are a sign that the environment of your urinary tract, microbiome, and immune system is out of balance. They are defined as 2 or more UTIs within 6 months or 3 in a year. Up to 60% of women will experience a recurrence of a UTI within a year of the first infection, and 1 in 3 experience a UTI before the age of 24.

 

As a Naturopath specialising in vaginal health, I commonly treat recurrent UTI’s, and understand how debilitating it can be. I am always looking for the WHY behind the recurrent infections (no band-aid approaches here!) so we can treat the UTI for good.

 

The Symptoms

Urinary tract infection, including recurrent or constant UTI symptoms, often include:

·      Burning or pain on urination

·      Increased frequency of urination, often passing only a small amount of urine

·      Pain or uncomfortable pressure above pubic bone

·      Pain in the lower abdomen, or lower back

·      Cloudy or strong smelling urine

·      Fatigue/tiredness

 

What’s really happening with recurrent UTI’s?

Up to 80% of UTIs are caused by bacteria, most often E. coli, which cling to the bladder lining and triggering inflammation. When infections keep coming back, it’s often about:

·      The bladder lining becoming sensitised or irritated

·      Changes in the vaginal and urinary microbiome that allow “bad” bacteria to take hold

·      Underlying structural, hormonal, or lifestyle pressures on the pelvic area

The vagina and urinary tract are neighbours, and the microbiome of one directly influences the other. When protective Lactobacillus species decline in the vagina, the environment becomes more alkaline and hospitable to pathogenic bacteria, which can then travel up the urethra and into the bladder. This is why recurrent UTIs often go hand‑in‑hand with vaginal imbalances, hormonal shifts, or even post‑menopausal changes

 

UTI symptoms with normal results

Some people have textbook‑like UTI symptoms, but repeated swabs or cultures come back negative or “normal.” It really does depend on the type of testing used, and if you were symptomatic at the time of testing. but it’s often from immune dysregulation, local oestrogen‑tissue changes, and microbiome instability, rather than just an acute infection.

 

Why recurrent UTI’s keep coming back

From a clinical perspective, there are several overlapping pieces that keep UTIs circling back:

·      Bladder biofilms and bacteria
Some bacteria can embed themselves in the bladder lining, forming a protective layer known as biofilms. These are harder for antibiotics and the immune system to reach, which can mean symptoms damp down after a course, only to flare again weeks later. I will always go in with some “biofilm busting” herbs and supplements!

·      Microbiome imbalances
The gut–vaginal–urinary axis is real. Overuse of antibiotics, stress, diet, or hormonal fluctuations can disrupt this ecosystem, reducing good bacteria and allowing bad bacteria to thrive. I normally recommend vaginal microbiome testing here (organised via our clinic) and urinary culture to get the whole picture for your symptoms and health.

·      Anatomical and functional factors
Things like pelvic floor tension, incomplete bladder emptying, constipation, or even sexual positioning can all influence how easily bacteria are flushed out with urine.

·      Hormonal shifts
Hormonal states like low oestrogen, whether from perimenopause, post‑partum, or breastfeeding, can thin the vaginal and urethral tissue, reduce local immunity, and shift the pH, making the environment more vulnerable to infection.

Treating recurrent UTIs isn’t just about killing the bacterial overgrowths, it’s more about creating an environment where infection struggles to take over again. This often looks like restoring the vaginal and urethral microbiome, breaking down biofilm, supporting the immune system, prescribing specific antimicrobial herbs, and understanding your hormones and nutrient levels to formulate a treatment plan that’s specific for you.
